Nemours is seeking an Ambulatory Clinical Documentation Specialist to join our Corporate Coding Integrity team in Wilmington, Delaware.

To keep pace with the changing health care environment and build a healthier future for our children, Nemours has embarked on a multi-phase expansion to create a children's hospital like no other in t! he region. Upon completion in June 2014, the hospital will inc! lude private patient rooms, a new Pediatric Intensive Care Unit, and a new expanded Emergency Department.

Review clinical documentation for opportunities to improve physician documentation as it relates to ICD9/ICD10 coding and possible gaps in the clinical documentation. Communication with physicians, APN's, PA's and residents to validate specific documentation.. Provide education as needed. Demonstrate and apply knowledge of the American Health Information Management Assoc standards of clinical coding. Serve as a resource to the surgical and ambulatory coders. Support development of templates, databases and other tools to support accurate documentation.

1. Review ambulatory medical records. Analyzes documented clinical diagnoses and compares this information to selected ICD9/ICD10 diagnosis codes. Identifies potential gaps in provider documentation.

2. Communicates with providers to validate observations and makes recommendations for appropriate! documentation.

3. Demonstrates basic knowledge of AHIMA and HIMS standards of coding and applies these guidelines to ongoing evaluation of the ambulatory medical record documentation.

4. Serves as a resource to physician coders (surgical & clinic) to ensure coding compliance with the ICD9/ICD10 coding guidelines.

5. In collaboration with management & physician leadership, provide educational programs to support medical record documentation.

6. Identifies trends and/or opportunities to improve clinical documentation. Presents such information and potential solutions to department Director.

7. Extensive knowledge of medical terminology, anatomy, physiology, pharmacology and disease processes.

8. Performs all other duties as assigned by management.

Must be a certified professional coder through the American Health Information Mgmt Assoc (RHIA, RHIT, CCS).

Minimum five years hands-on experience in clinical! coding.

Thorough knowledge of CMS' coding and documentation g! uidelines.
